If this is the venerable Kurzweil system that goes back a decade, it was
actually based on keywords. The doctor said a keyword such as "normal", and
the complete paragraph describing "normal" (already stored in the system)
was inserted in the document with patient-specific content folded in. The
rest of it used voice recognition for a fairly limited vocabulary, trained
to the speaker. This may have changed since 1992, but it was very efficient
since it required training only for a field-specific vocabulary. The one I
reviewed was for radiology.
