I've used the Boss TU-2 many a time. It's a pretty failsafe option for stage tuning.

If you want something for acoustic instruments, there's the Korg CA-30. I've used this often on my acoustic guitar with good results.

Then again, if your an iPhone user, maybe you want to look into the Cleartune app. A friend of mine started using it recently; haven't heard anything negative about it so far.

If you're looking for something rack-mounted, the Korg DTR 1000 is widely used:
